  • 1.  ICD's end of life deactivation

    Hi 

    I wanted to ask communities about real life patient scenarios of withdrawal of care and programming off ICD therapies in home setting when patient is unable to come into clinic or ED. 

    I reviewed the 2010 consensus statement and noted this addressed. However the question came up in our group about possible legal liability,  Real or not

    Good morning,

     

    I would be interested to hear from some of our industry partners.

    At our local center, which is a federal center (VA), we collaborate with our local device representative.

     We provide a written order from a physician, that allows them to set up a home visit, and they confirm with us once it is done.

    We have essentially the same process as Jessica regarding turning off device therapies. It (surprisingly) only comes up a few times/year.

     

    Have a discussion with the pt (if capable) and family and document. Most companies need a written order specifying the programming changes. Pre-COVID, most companies allowed our local reps to go to house to turn off device. During COVID and now, at least one of the companies require pt to be at a doctor's office or facility with exceptions if the pt can't be moved.

     

    I also look forward to the comments from any industry partners in the community.
