  • 1.  How we address psychological stress from multiple ICD shocks

    in the July 8, 2022 Heart Rhythm, Dr. Lauren Schneider's excellent study of post traumatic stress disorder in young ICD patients and their parents brings to mind that the same factors and stressors affect our adult patients.  We have all  provided care for patients who have received multiple ICD shocks, some of which were appropriate and some that were due to device/lead  failure or programming issues.  We now recognize that the burden of the ICD shocks is very analogous to PTSD.   Are we adequately addressing the psychological needs of the multiple shock patient?   Are we fortunate to have resources in-house?  Do we know of outpatient psychologists and therapists to whom the multi-shock patient can be referred?  Is there a way for us to adequately prepare for our next meeting with a patient who has suffered multiple shocks?

    Hi Paul

    Thanks for bringing up this extremely important topic.  With newer device technologies, diagnostics and alerts, shocks have become "less of a problem" than the early days when we were dealing with "shock boxes". However, as you identified, shocks and PTSD are still a component of the device landscape, and we should be prepared to help our patients suffering with this.  

    I would suggest you google "Sam Sears, PhD" and you will find a plethora of studies and information on shocks and PTSD in the device patient population.  Sam had dedicated his career to this topic.  

    Perhaps this would be a good topic to discuss at our next Journal Club and invite Sam to join us!

    Brilliant idea.  I have met and spent time with Dr. Sears as well.  This would be so wonderful as he is an expert on the topic and such a great speaker.
