Our protocol for missed appointment is to call them... if we make contact we reschedule over the phone. If we don't reach them we mail them a letter with a new appointment marking in the appointment note (missed (date) RSC x1.
When appointment is missed 3 consecutive times we send a certified and regular letter informing them that they will be dismissed from our clinic if they fail to be seen in our clinic in 30 days

We have a protocol that we call the patient initially, call again 1 week later, and again 2 weeks later. If no call back within a week after the 3rd call, we sent a certified letter that if we do not receive a response within 90 days, we disconnect them.
