  • 1.  Device clinic records

    Does anyone know what the standard of care is for record keeping of device interrogations? What data needs to be saved or imported into the EMR? Reports from the programmer or remote systems can be many pages long. We are in the process of transitioning to Epic so I'd like to facilitate appropriate record keeping, currently we just have paper charts and I do a summary report in the EMR.

    Hi Britt, 

    We use a third party group, Geneva, who collates all the remote transmissions from various companies and merges them into EPIC.  For clinic device checks, we save all as PDFs with on a USB or via the Medtronic iPad then upload them into Geneva and they are carried over to EPIC once signed.  I believe there are other companies available for similar use, MURJ is one that MANY institutions use.  Hope this helps!

    Yes that is very helpful, thank you. Do you know what data is saved? some of the reports are many pages long and fairly redundant. I have typically just been saving the summary reports and maybe a small sample if there is any arrhythmia. I haven't been keeping the trending data because the device does a good job of saving it for us. I just want to make sure I am meeting the standard of care and sending the appropriate reports to be scanned into Epic when we get it.

    Hi Britt,
         We are using Murj and have been for about 5 years. Once the document has been signed by the reviewing provider, the entire device report is sent electronically to our EMR. When the document is opened in the EMR, the first page is the summary report that was written by the device tech. Scrolling down will get to the rest of the device report, if anyone wants to see all the information. Our general and interventional cardiologists are not really interested in the full device report, but the EPs and HF docs sometimes find it useful to delve deeper if they want more information. I find that this creates an easy way to have all the info available for those who want to it, without creating tons of scrolling for those who just want the summary. This also makes it much simpler for our device department,  as they do not have to try and decide what to save for the EMR for each patient.  Hope this helps.

    We have been with Epic for several years now.  We scan our provider interrogations in the office and upload them via Media Manager in Epic.  For in clinic checks and remotes we utilize Murj.  Our in clinic checks are uploaded using a USB drive and our remotes are sent to Murj from the various vendor websites and processed by our staff.  We have found Murj to be an excellent alternative to our previous use of Paceart.

      Britt,
    The Iowa Heart Center device clinic saves all of the in-person evaluation- The PDF includes initial,and final programming, all diagnostic trending,and a sampling of event EGMs (when in the office and all when done via remote).
    When in-office threshold evaluations are done, we also save (from the programmer)  the initial rhythm strip, sensing strips, at least one example of each lead stimulation threshold test, any troubleshooting rhythm strips and the final rhythm (only if there were any programming changes that would affect the final rhythm). This is very helpful if there is any question on the functioning of the device as well as proof of what was done. [Quality audits require proof of results.] We use the past years trending information at times to review for subtle changes over several years. Once the device evaluation write-up is reviewed and signed by a physician in our Paceart system, the evaluation report PDF is automatically sent to the EMR with the PDF of the device data from the programmer or the remote transmission.
