Billing for in-clinic and remote follow-up for patients with 2 devices

Colleagues, would you please share your practice regarding billing for patients with 2 active, implanted devices. A SQ ICD and a pacemaker. We complete a report for each one. Do you bill for both or just one? Thanks.

Before I retired, we always completed a report for each and submitted a bill for each. I believe this is still valid.

We bill for each one separately with separate report for each.

    Hi Linda,

We billed each device, separately, with accompanying reports.  We usually have different dates to make it simple.

Thanks so much for responding, Martha. Different dates will work well for remote monitoring. What do you do for an in clinic check (full testing) of both devices on the same day?

In office, we bill both devices. They have separate codes and separate diagnoses to associate with each (ie sinus node dysfunction for pacer and vt for ICD) so usually not a problem.

    Hi Linda,

As everyone else has mentioned, I bill for both devices in office and we try to stagger the remotes so they're due a day or two apart from each other.  As long as the documentation is there, it's generally not an issue.  The only time it's been an issue is when it wasn't documented appropriately.

    Good morning,

Would this include patients that have an ICM implanted and then undergo implantation with another device?  Normally, the ICM is removed but we have had instances where patients have both.  We normally don't bill for the ICM.

Thanks in advance for the information,

    Michael,
If the ICM continues to be interrogated and monitored remotely, you could bill for it. If you no longer process any remotes or write up any documentation for it, then you would not bill for that device.
