When Medtronic Home Monitors are returned to your clinic, do you have the patient fill out a form telling you why it's being returned to the clinic?
Recently, we recognized, our Device Clinic receives a large volume of returned Medtronic Home Monitors- (24950's mainly). The HM's, are dropped into the clinic and then shipped back to MDT. We realized that there was no monitoring of these returns and many of them contribute to the increased number of patients on the Disconnected HM List. We were considering a simple half sheet of paper the patient fills out, so we can have accountability of these returns and their relationship to the web site. This sounds so archaic.
Can you all share how you accept home monitor returns and what you do to help your clinic website get updated after the return?
